One Call Does It All
At Hartselle Medical Center we are committed to providing services and options to our physician partners to ensure your patients who are hospitalized receive comprehensive care during their stay with us. One such service is our Hospitalist Program.
To enroll your patients in the Hospitalist Service just simply call the Hartselle Medical Center Access Nurse. Once your patient is discharged they will return to you for follow-up care. The hospitalist who cares for your patient during their hospital stay will communicate with you as needed and also provide a discharge summary.
Contact Hartselle Medical Center’s Access Nurse at 256-612-2959 to reach the Hospitalist on call. The HMC Access Nurse is available twenty-four hours, seven days a week and can reach the hospitalist on call to admit your patients into the HMC Hospitalist Program.
Continuous, Individualized Care
Once your patient is admitted to the hospital he/she can be seen by our on call hospitalist either in the ER or in their room. You can communicate with the hospitalist as often as you would like to discuss your patient’s health care needs. Hospitalists are able to provide continuous, individualized care for your patients when they need it most and are available to your patient around the clock, helping you to provide the best possible care for your patient.
More Timely Care
If a situation arises, a hospitalist can make a prompt decision or modify treatment on the spot.
We are here to assist you.
HMC Hospitalist Service is here to assist you and your patients. Once the patient is released from the hospital, they return to you, the primary care physician, for ongoing care. A discharge summary will be available to you before their follow up visit to your office.
For the smoothest transition to inpatient care we recommend you communicate with the hospitalist as you refer your patient for admission.
- To reach the hospitalist on call, please contact the HMC Access Nurse at 256-612-2959.
- Once your patient is admitted and in their room, an HMC Case Manager will explain the hospitalist program and provide patient education materials that helps explain our Hospitalist Service.
- During regular office hours, please send your patients to registration for admittance.
- After hours or on the weekend, please send your patients to the HMC Emergency Department to be admitted into the hospitalist program.
